Establishing As A New Patient
You can schedule an appointment to be established as a new patient. On this visit we will obtain vital signs, BMI, review past medical history get a list of your current problems, medications, obtain baseline labs and perform a head to toe physical evaluation. Since we are obtaining baseline labs we do request that you come fasting and schedule a morning appointment. We will not diagnose any immediate issues you may have, that will come at the follow up appointment. Copays are required for these visits. **We do have a no show/cancellation policy that we will enforce for all new and existing patients. This includes a $50 charge for cancellations within 24 hours of your scheduled appointment and no shows. We will require valid card information to secure your appointment when you schedule. **Our new patient labs are NOT considered 'preventative care labs.'
We do require you to be an established primary care patient in order to participate in our weight loss program. This means that we will assume ALL of your primary care from your previous provider and will be your sole primary care provider moving forward for continuity of care. We will request records from your previous provider. We cannot manage primary care in patients living outside the state of Alabama or long distances from our office. In order to qualify for the weight loss program (that is not covered by insurance) you have to meet Alabama Board of Medical Examiners requirements for treating weight loss which include:
On this visit we obtain vital signs, BMI, vision screening, review past medical, social and family history. We review medications and current problem list. We will perform any cognitive screenings that are required. We update screenings and vaccines. We will not address any new problems at these visits or diagnose any issues you have on these visits. If you have emergent issues we will reschedule your wellness exam and perform a sick visit instead. Copayment is not required for annual evaluation however if the exam changes to a regular sick visit it is subject to normal copayment.
These visits are scheduled to follow up on existing problems and diagnosis'. Typically these visits include management of chronic disease such as metabolic syndrome, hypertension, diabetes, and dyslipidemia. These visits are subject to normal copayments, refer to your insurance if you have questions about copayments on follow up exams. It is a visit so a copay is due for the service performed during the visit.
These visit include obtaining blood for testing as requested by one of our providers. These visits include phlebotomy to obtain your blood samples. Please call the office and ensure you do not need to be fasting before you schedule this visit, as coming at the wrong time of day or not fasting can cause rescheduling of the appointment to the appropriate time of day and fasting. If you are on topical hormones such as creams or drops do not apply the drops the morning of your lab appointment. Any labs for cholesterol, or blood sugar levels also have to be drawn fasting. You will not be seen by the provider but these do have to be scheduled. We do not draw labs after 4pm.
These visits include administration of your chosen wellness injection. You are responsible for the amount of the wellness injection selected and these are not billable to insurance benefits.
